ICE Forms

Produce the request form using ICE requesting system or order comms. Print or write date on the form, not forgetting to state this on the labels also.

Detach labels from the form depending on where you have decided to have them integrated. Many hospitals/NHS trusts may decide to choose their own layout.

The Detached labels are to be applied to the sample tubes

---

Stick the labels onto the sample tubes as shown

---

Detach the label half of the form

---

Dispose of the detached half as you would any other paperwork containing patient identifiable data

The form is to be inserted into the specimen bag, then the sample is placed into the smaller pocket on the specimen bag. Once sealed the form and sample are ready to be forwarded to the laboratory.
